Par is the score a scratch golfer is expected to take on the hole. Add the pars together and you get the par for the course, usually 70 to 72.
Stroke index ranks the holes from 1 to 18 by difficulty, where 1 is the hardest. It decides which holes a player takes their handicap strokes on. A player with a course handicap of 6 receives a shot on the six holes indexed 1 to 6, so their net score on those holes is one less than they actually took.
OUT is the total for holes 1 to 9, IN is holes 10 to 18, and TOTAL is the two added together. In a Nassau these are the three bets: the front, the back and the whole eighteen.
Marker is the person who keeps your card in a competition, and Player is you. Both sign it at the end. In a friendly round nobody bothers, but the boxes are there.

Yes. Six player columns covers a fivesome or sixsome, and the extra blank rows are useful for a team total or a running side bet.
Plenty of people like one in the back pocket, and it is handy for an outing where not everyone has a phone. The difference is the maths: a paper card records what happened, but you still have to work out the skins, the presses and who owes who at the bar. Big Fairways does that part for you while you play.
